» RenViva offers traditional 3 day or 5 day/week dialysis therapies.
» Treatments performed in the Nursing Home (NH) are convenient for patients and NH personnel. The location of a dialysis treatment helps to improve the length of stay for both rehabilitation and skilled nursing home stays.
» RenViva personnel are experienced renal nurses and patient care technicians which improves the communication between the NH personnel and the dialysis provider to achieve better patient’s outcomes.
» RenViva provides state of the art medical equipment and EMR systems that can be accessed by the NH personnel. The coordination between the patient care records of both providers improves communication especially during regulatory surveys.

Transitional Cardiology in the post acute care bridges the gap of providing cardiology from the hospital to the skilled nursing facilities and even to the patient's home. This is accomplished by having cardiology on site with 24/7 access to cardiology providers. We enhance the communication between the patient's outpatient cardiologist and the hospital at the facility where the patient is getting rehab. By partnering with the hospitals in the area, the idea is to implement their protocols, utilize and coordinate with their CHF clinics, and most importantly, make sure all patients return to their outpatient physicians and offices. We accomplish all of this by being cost effective to the patient, facility, and the hospital. This also translates to reducing all cause readmissions for the hospital and skilled facility.
